List of Documents Required for Death, Registration and Vote of No Confidence Cases of Local Governments Representatives

Government of the Punjab, Local Government & Community Development Department has issued Notification on 13-03-2019 in connection with List of Documents Required for Death, Registration and Vote of No Confidence Cases of Local Governments Representatives.  Detail is as under:

The undersigned is directed to refer to the subject noted above and to state that in future all the causes on the subject would be forwarded to the LG&CD Department along with the following documents duly attested by the Deputy Directors/ Chief Officers (affixing original signature and stump) of the concerned local Government, otherwise the  case will not be entertained:

1 Copy of CNIC In the case of death of Chairman copy of CNIC of Vice Chairman as well Copy of CNIC In the case of Registration of Chairman copy of CNIC of Vice Chairman as well Requisition moved by the members for convening meeting to discuss Vote of No Confidence
2 Copy of computerized death certificate Copy of hand written registration Notice issued by DDLG for meeting to discuss Vote of No Confidence alongwith evidence/ means of communication of notice
3 Copy of result notification Copy of Result notification   Attendance of candidates who participated in the meeting to discuss motion of no confidence
4 ----------------------------------------- Copy of House Resolution Proceedings of the meeting of the House
5 ----------------------------------------- ---------------------------------------------- Declaration under Rule 8 of Punjab Local Governments (Vote  of No Confidence) Rules, 2018 regarding passing of Motion of No Confidence by 2/3 majority.
